Nomination Preparation:
Candidates are required to prepare the following: (1) a fully completed self-nomination form, endorsed by a MASM life members and seconded by another MASM life or ordinary member; (2) a photocopy of Identity Card/ Passport; (3) a photocopy of the highest academic achievement, awards or certificates, and (4) a recent half-body portrait photograph.

Submission of Documents:
Nominees must email the soft copies of the nomination form and other relevant documents to secretary@malaysiansportsmed.org. Additional, verified hard copies of these documents must be mailed to the following address:

Evaluation Process:
All nominations will be thoroughly reviewed by the FMASM Subcommittee, consisting of five MASM Council Members: the President, Vice President, Secretary, Treasurer and one Ordinary Council Member.

Recommendation and Voting:
The Fellow Subcommittee is responsible for making recommendations regarding the nominations to all MASM Council Members, who will then vote on these recommendations.

Final Decision:
The decision made by the MASM Council is conclusive and irrevocable.

Announcement and Awarding:
The MASM Council will announce the successful nominee during a General Meeting. Approval from the life and ordinary members present at the General Meeting is required, with a simple majority vote needed. Once approved, the title of Fellow of Malaysian Association of Sports Medicine (FMASM) will be formally awarded to the nominee at the next major MASM events, such as annual conference, congress or scientific meeting.
